Returning Players: Enclose a 1" x 1 1/2" photo and a check for the appropriate amount.
New Players: Enclose a copy of the birth certificate, a 1 x 1/2" photo, and a check for the appropriate amount. Form must be signed in IMPORTANT MEDICAL and LIABILITY RELEASE box in blue ink.
DONATIONS: Your $30 donation will help a child in need play soccer. Please check the donations box on the registration form and enclose check.
Instructions:
- Click on the link above. Download the file. We will need (2) completed
copies per child with an original signature. It must be in blue ink.
- Division 4 (Recreational), one (1) copy of child's birth certificate
for age verification if the player did not register last year.*
Division 3 two (2) copies of child's birth
certificate for age verification. If the player is trying out for Class
III, please register in advance. After tryouts, the full registration
fee will be refunded if the player will be playing for a different club.*

*Birth certificate is the official State/County issued certificate, not
the one issued by the hospital.
Fees
- Registrations received before June 1st are $125/player (up to 2 children). Third child from same family is $85.
- Registration for U6 players is $85.
- Registrations received after June 1st must include a $25 late fee.
- Registrations received after August 1st will be placed on waiting lists.
- Scholarships are available to qualified applicants, limited number available.
- NO REFUNDS will be given afer September 1, 2010.
Refund requests must be:
- in writing
(Include child's full name, date of birth & age group.)
- mailed to: MCYSC, P.O. Box 668, Soquel, Ca 95073.
- postmarked by August 31, 2010.
- please include a self-addressed stamped envelope.
Requests received before August 31st will be refunded less a $32.00 registration fee.
